  • This checklist must be completed by the oil company's representative prior to commencement of transfer operations.

SPILLAGES MUST BE CLEANED UP AND REPORTED TO THE OTAGO REGIONAL COUNCIL

  • Fines under the Maritime Transport Act 1994 and Resource Management Act 1991 are a maximum of NZ$300,000 (individual) or $600,000 (Company) PLUS expenses and costs incurred in removing and cleaning up the oil.
  • The completion of this form should be considered as part of the Tier 1 response plan as required by Marine Protection Rule Part 130B.
  • No action whatsoever by the transfer operator in conjuction with this checklist shall affect the ship's obligations and/or liabilities in any way.

Declaration

ENSURE THAT:

  • When transfer ceases or is temporarily stopped, valves are closed.
  • Relevant safety procedures are maintained throughout the transfer.
  • During transfer, competent persons are in continuous attendance at valve manifold on wharf.
  • During transfer, competent persons are in continuous watch on deck and in engine room.
